Cause of death: Non-hostile, died of illness/injury. Burns/Smoke Inhalation. Burns. Individual died as a result of injuries received while a crew member of a military vehicle on a military mission when the main weapon of the vehicle was fired with the breech open and a fire started. He was admitted to a military medical facility and later expired. / Loader on a tank (Sheridan) on training exercise.  Main gun tube fired with the breech open and a fire started.  Individual admitted to 24th Evacuation Hospital on 1 Nov 1969 at 1730 hrs and placed on Very Serious Injury (VSI) list on 1 Nov 1969 at 2030 hrs.  Individual was later transferred to Japan on 3 Nov 1969 where he expired on 15 Nov 1969.  Incident occurred on 1 Nov 1969 at 1530 hrs.
